Job Overview

Asylum Hill Family Medicine , a member of Trinity Health Of New England Medical Group, is seeking a LCSW Social Worker to join our Family Medicine team.

The Licensed Clinical Social Worker will collaborate with patients, families, and the medical team to promote recovery and optimal well-being. The ideal candidate will demonstrate a commitment to patient-centered care through scientific inquiry, compassion, advocacy, education, and teamwork.

Responsibilities

1. Perform psychosocial assessments, counseling, crisis intervention, case management, patient support, and education.
2. Connect patients to community resources.
3. Support patients and families, advocating to ensure appropriate care.
4. Communicate effectively with patients, families, and the clinical team to develop safe care plans.
5. Collaborate with care team members, including physicians, therapists, nurses, case managers, and spiritual care providers, to update care plans as needed.

Minimum Requirements

1. MSW degree from an accredited School of Social Work.
2. CT State Social Work Licensure (LMSW or LCSW preferred).
3. Preferred certifications: Case Management (CCM) and Continuity of Care Advanced (ACCC).
4. Knowledge of Medicare, Medicaid, and managed care regulations and trends.
5. Experience with utilization management, home care, community resources, and rehabilitation is beneficial.
6. Bilingual (Spanish) preferred.
