Nurse Practitioner – Palliative Care

Location: Traverse City, MI

Company: ATC Healthcare Metro Detroit- Contact with Gentiva Palliative Care

Job Type: Full-Time | Days | Some On-Call Required

Travel: 10–20% local travel for patient visits, training, and team meetings (mileage reimbursed).

What You’ll Do:

• Deliver in-home and community-based palliative care, including diagnosis, treatment, and follow-up in collaboration with our Palliative Medical Director.
• Develop and manage individualized care plans based on comprehensive assessments and patient/family goals.
• Coordinate services across the care team—including Social Workers and Spiritual Care Coordinators—to address physical, emotional, and spiritual needs.
• Prescribe medications and order diagnostic tests and equipment within scope of practice.
• Participate in rotating call coverage and provide responsive, high-quality care.

What You’ll Need:

• Master’s degree in Nursing (MSN) and current Nurse Practitioner licensure (Family or Adult NP preferred)
• Minimum 2 years’ experience as an NP, with working knowledge of palliative care or serious illness management
• Current Part B Provider status, DEA license(s), and collaborative agreement (if required by state law)
• Excellent communication, documentation, and critical thinking skills
• Reliable transportation, valid driver’s license, and auto insurance

Preferred:

• ACHPN Certification (Advanced Certified Hospice and Palliative Nurse Practitioner)
• Experience with Part B billing and point-of-care software
