What You'll Do:
Imagine yourself delivering compassionate, patient-centered care in the comfort of patients' homes and through virtual visits. You'll conduct comprehensive assessments, including annual wellness exams, counsel and educate patients and families about available benefits and programs, and meticulously document all aspects of patient care. Collaboration with primary care providers, specialists, and ancillary services will be essential to ensure coordinated and holistic care. You will prescribe medications, order tests, and collaborate with the patient's physician. You will also facilitate closing gaps in care by educating patients about preventive monitoring and working with physician practices to schedule diagnostic testing, assist patients with enrolling to access educational videos and participate in integrated care team meetings. Your focus will be on reducing avoidable hospitalizations, maximizing quality of life, and prioritizing patient health and satisfaction.
What You'll Bring:
Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license - your essential credentials!
Board certification (NP: ANCC/AANP; PA: NCCPA) - your professional recognition!
Current and unrestricted DEA certificate - your authorization to prescribe!
Basic Life Support (BLS) certification (company will support completion during onboarding) - your commitment to life support skills!
2 or more years of direct patient care experience - your proven track record!
Managed Care/IPA/Health Plan experience (preferred) - your familiarity with managed care!
Experience conducting annual wellness visits or similar comprehensive visits (virtually or in-home) - your specialized skills!
Ability to work independently and practice autonomously - your self-direction!
Access to transportation, a valid driver's license, and car insurance - your ability to travel!
Proficiency with medical instruments and equipment - your technical skills!
Knowledge of computer-based data management programs and information systems - your tech-savviness!
Excellent communication skills (written and verbal) - your ability to connect with others!
Sound understanding of federal and state regulations (HIPAA and OSHA) - your commitment to compliance!
Bilingual (English/Spanish) skills (a plus!) - your ability to reach a wider audience!
